Promotion Offer Limit:
Each individual is entitled to only one promotional offer. Any attempts to redeem additional promotional offers may result in the forfeiture of such offers. If you choose to make additional purchases beyond the initial promotional offer, a 5% deduction will be applied to your refund to cover transaction costs associated with processing. This deduction is non-negotiable and will be automatically applied to any refunds issued under these circumstances.
Appointment Cancellation Policy:
Cancellations must be submitted in writing to [email protected] at least 24 hours before the scheduled appointment. Phone calls and voice messages will not be accepted as valid forms of cancellation. Failure to provide written notice may prevent another patient from reserving that appointment time. In cases where written cancellation is not provided as required, the full amount paid for the appointment will be forfeited. Any refunds granted due to extenuating circumstances are at the sole discretion of Injury Health Center.
Late Arrival Policy:
Punctuality is important to ensure that all appointments run smoothly and on time. If you arrive more than 10 minutes late to your scheduled appointment, we reserve the right to reschedule your appointment to another available day and time. While we will make every effort to accommodate you, please understand that this may not always be possible due to our scheduling constraints. No refunds will be provided for appointments rescheduled due to late arrival, and any rescheduling will be subject to availability.
